MANCHESTER -- Police have identified a man who died following a shooting Monday afternoon.
The shooting took place at 158 Eldridge St. Police said Joseph Swan, 45, was found with a gunshot wound to the head and was taken to the hospital where he was pronounced dead.
Police said this is an ongoing investigation and additional information will be released as the investigation continues.
